    This is to certify that B/O Vatika Sharma is currently admitted in Neoclinic via IPD No- 2317435 from 10/1/24. Child is diagnosed as a case of Extreme preterm 26 weeks/ELBW/ AGA with RDS/ Grade II IVH/Neonatal Sepsis/Respiratory Failure/Thrombocytopenia/ Shock/Pneumonia/Right lung collapse / BPD/Recurrent extubation failure/Anemia/ Hyponatremia/AKI/Osteopenia of Prematurity/ Perianal excoriation.
